Configure action button section to accept name and email address

I’ve been using this theme for a client and it’s very clean and nice to use but as expected questions are starting to arise about customisations. I’ve been able to address most of them by editing the CSS but there’s one that I haven’t been able to tweak and haven’t found any documentation in the forum about either.

How would I configure the Action button section on the home page to provide two fields (name, email), have those fields submitted to an email address when the submit button is clicked and have a custom notification display (i.e. ‘Thanks for submitting. We’ll be in touch soon’)?

Any help much appreciated!

That has nothing to do with theme itself and you should code your own solution or use email subscription plugin such as Mailchimp for WordPress or something even simpler such as subscription module which comes with Jetpack plugin.
