I have installed and activated both Unique Headers and WP Display Header plugins per a previous support thread for Shapely, but my active header image still does not show up.

I hope you are well today and thank you for your question.
Do not just install the plugins but configure them as describe don those plugin pages and do not activate both plugins at a time which will conflict with each other.
