I’d like to insert a widget asking people to sign up to our email list, so it’s visible above the fold, and also visible on the top of page of the mobile view.

We have a shortcode [USM_plus_form] we can use.

Please help figure this out.

Also, is there a way to add a third button?

I hope you are well today and thank you for your questions.

We have a shortcode [USM_plus_form] we can use.

Then just add that shortcode in the text widget provided by following plugin and use that text widget in the widget area of your site.

Also, is there a way to add a third button?

Currently there isn’t any widget option to achieve this. You have to develop custom code to achieve this.
