Add form in Editor

I wanna add 3 text fields “Address” , “Email” , “Contact No.” to replace the button “Learn More”.
When User filled the form and click “submit”, data will send to my email.

I’ve already install “Contact Form 7”. but I don’t know how to make it.
Can you teach me? Am I need to edit coding on “Contact Form 1” and also “front-page-bottom-header.php”?
