Is it possible to get a contact form in place of the social icons under the Contact Us Section?

The social icons and information is far too small leaving lots of empty space and would be handy to have a small 4 part form of Name, Email, Subject, Content. Can this be done? Rather than a huge form across the entire section.

it wont allow someone to fill in the fields… any help greatly appreciated.
