How to use contact form?

I have found one contact form template that i would like to use for a website that i am currently building.
It’s Contact Form v5.
Question is how can i use it?

Hi there

You can implement it on your website if it’s an HTML website, you also need to implement PHP form to process form data.

Please note, that working knowledge of HTML is necessary to make it work.
